Florida Sinus Institute Resource Center
The Florida Sinus Institute Resource Center will help you get directions to our office,
find our suite, download and print new and existing patient forms so you can fill them
out prior to your scheduled appointment with Dr. David Greene.
Our Office Location
Our office is located at 1112 Goodlette-Frank Rd N, Suite 203 in Naples in the
Colonial Square office complex. Please refer to the interactive map below or the
directions for getting here on the right side of this page.
New Online Patient History Form
When coming to our office, the more we know about your medical condition, the better we can provide care. Please print the following patient history
form so that you can provide us with the most accurate information possible and save time while checking in. You can fax or e-fax your forms to 239-308-0894
or print and bring the documents with you to the scheduled appointment.

Directions to Our Office
Dr. David Greene is located in Colonial Square
just south the Naples Post Office
on Goodlette Frank Road.
Coming from I-75:
Take exit 105 (Golden Gate
Parkway) and turn West as if you
were going to the Coastland Mall.
Continue driving until you reach
Goodlette Frank Road. Turn left
onto Goodlette Frank Road. You
will pass the Naples Zoo and the
Post Office on your left side.
Immediately following the Post
Office is the turn-in to Colonial
Square. Turn left into our complex,
make your first left and proceed
to the end of the lot and turn right.
Our office is immediately on the
right.
Coming from Downtown:
Take US41 East away from 5th
Ave. South. Turn left onto
Goodlette Frank Road. Continue
for approximately one mile. When
you pass The Commons on your
right side, our complex is the next
parking lot. Turn right into our
complex, make your first left and
proceed to the end of the lot and
turn right. Our office is immediately
on the right.
Inside Our Building:
Once you have entered our building,
you can proceed to the second floor
using the stairs or the elevator.
Once you are on the second floor,
follow the signs for Dr. David Greene and the Florida Sinus Institute through the hallway
to our front door and enter our waiting area.
